What to bring

Bring the following to your appointment:

• Insurance card(s)
• List of current medications
• Current pair of eyeglasses
• Contact lens patients, please wear them and provide the brand, power, and base curve.

Please arrive 10-15 minutes early if you are a new patient in order to complete paperwork. You may also print out the paperwork and bring it to your appointment.

Rescheduling, Canceling, or Late Arrival

Patients are required to give 24 hour notice for canceling or rescheduling an appointment. This allows us enough time to offer the appointment to another patient. Less than a 24 hour notice is considered a missed appointment. Patients who miss 2 appointments are required to provide a deposit in order to schedule any future appointments.

As a courtesy to all our patients, we strive to run on time. We reserve the right to reschedule or cancel patients who arrive 15 minutes or more later than their scheduled time. If you are running late, please notify our office as soon as possible.

Any patient under the age of 18 must be accompanied by a parent or legal guardian.
