What to Bring

Bring the following to your appointment:

• Insurance card(s)

• List of current medications

• Current pair of eyeglasses

• Contact lens patients, please wear them and provide the brand, power, and base curve.

Please complete your paperwork online, this is highly recommended! New patients, if you are unable to complete online forms, please arrive 10-15 minutes early to complete your paperork in the office or you may print out the paperwork and bring it to your appointment.
